Crestview Nursing and Rehab Center is a 65-bed nursing and rehabilitation facility, located in West Branch, Iowa. It is one of the more than 50 locations of Care Initiatives, one of the largest senior care providers in the state. It started operations in 2000, and provides personalized service and team-oriented care program. It has rehabilitated and assisted many residents in their return to home, or a more independent living situation. Its rehabilitation services, which are also offered on an outpatient basis, include physical, occupational, and speech therapy.
