Symphony of Crestwood

Crestwood Care Center offers various health care services. The center offers a professional and interdisciplinary assessment of each resident's health care needs. It develops a plan of care based on those needs, with the resident's wishes, abilities and directives as the central theme. It also offers provides social services, recreation therapy and spiritual support as needed or desired by each resident and to treat each resident. It offers various nursing services, such as coordinated care planning, monitoring vital signs, health status, weight and nutritional intake, medication and pain management, and assistance with bathing and other activities of daily living. It additionally offers wound care and dressing changes, feeding tube administration, administration of oxygen as needed, palliative and comfort care for terminal illness, discharge planners and restorative aides for long-term care. It also offers rehabilitation services and recreational activities. Crestwood Care Center is located in Ogden, Utah.
