Located in St. Charles, IL, CovenantCare at Home and Hospice was formed in April 2011 and earned CHAP accreditation in October of that same year. Our urban landscape and proximity to three of the largest Covenant Retirement Communities allows us to service a broad and diverse population within the Chicagoland area. The ability to offer private duty, home health, and hospice services to the campus residents aligns with CovenantCares and CRCs emphasis on creating a continuum of care.

Disability Management Network, founded in 1990, offers a wide variety of rehabilitation and counseling services to companies throughout northern Illinois and southern Wisconsin. Its services include medical case management, vocational rehabilitation, on-site occupational health and total health package. Its vocational rehabilitation services include vocational assessments, testing, labor market surveys, job analysis and placement, and testimony and trial work. It provides early assessment and ongoing case management of employees eligible for short- and long-term disability benefits, and promotes a multidisciplinary approach to disability management. Its disability program is customized as per the needs of the company. Disability Management Network has offices in Oakbrook and Rockford, Ill.
