Community Support Servies

Community Support Services is a private, nonprofit service agency that provides and promotes services for people with developmental disabilities and their families to strengthen their independence, self-esteem, and ability to participate in and contribute to community life. It offers an array of services, including respite, case management, transitional living services, and supported living, employment and parenting services. Its case management staff works with the individuals and their families to identify and prioritize needs, and develop a service plan that establishes short- and long-term goals, and objectives for achieving them. The service plan addresses areas, such as information and referral, accessing home-based services, obtaining financial and medical benefits, securing medical and dental services, individual and family counseling, crisis intervention, recreational opportunities and many other services. It is located in Brookfield, Ill.
