Dr. Lauren Baker, DPT; evaluates and treats kiddos 2 months to 6 years in the Milwaukee area including any areas located 30 minutes from the Milwaukee neighborhood of Bay View. Older children can be treated, but hours of services may not work with school schedules. Payments via credit card, health savings account, or flex spending account all done via auto-billing post-appointment through our simple practice portal. Insurance reimbursement forms are available upon request. Services include: Free Introductory 15 minute call, In Home 60 minute Pediatric Evaluation $120, In Home 60 minutes Pediatric Treatment Session $120, and Online Parent Consultation $50. Telehealth options are available by request.
