Young-Jae Nam, MD

Sylvia Sakulanda graduated as a Nurse Practitioner from Walden University in 2016. She also completed Acute Care Gerontology Nurse Practitioner training in 2019 from Georgia State University. Sylvia has five years of cardiovascular experience as a provider. She has worked as a cardiac nurse for seven years prior to becoming a nurse practitioner. In her spare time, Sylvia likes to exercise and eat healthy. Her hobbies include out door running. She's signed up and looking forward to the 2021 Peachtree Road Race 10k.

Rebecca Chaffee, PA-C, earned her medical degree from Trevecca Nazarene University. She received her undergraduate degree in Exercise and Sports Science from The University of Alabama. Rebecca has 3 years of cardiology experience as a provider. She is board certified with the NCCPA. In her spare time, she enjoys spending time with her family and fiancé, exercising, and watching Alabama football. She is accepting new patients and welcomes most major insurance plans.

Dr. Douglas Miller has expertise in the medical and surgical treatment of a wide range of diseases and disorders of the ears, nose, sinuses, larynx, mouth, throat and structures of the neck, including chronic sinusitis, facial plastic and reconstruction, and head/neck cancers. Dr. Miller treats patients of all ages - infants through seniors - and has served the Walton County community since 2010. In his spare time, Dr. Miller enjoys spending time with his family. Prior to establishing his medical practice in Monroe, Dr. Miller proudly served as a medical officer at the U.S. Naval Hospital in Yokosuka, Japan.
