Floyd - Primary Care

The Floyd Cardiac Rehabilitation focuses on care for patients after a heart event. Our program uses exercise, behavior changes and education to help build a patients physical fitness. Build to return the patient to the highest possible level of activity, our program is unique to each Physician. The outpatient cardiac rehabilitation program located at Polk Medical Office Building on the Polk Medical Center campus.
