We are pleased that you have selected our office to serve your needs in the field of Orthopedic Surgery. It is our continuing goal to provide you with the most comprehensive and up-to-date health care possible. It is important that our patients become partners with us in their own ongoing health care. Should you have any questions regarding your treatment or office procedures, please express them to our trained professional staff. If you are a new patient, you will receive an information form and a medical history form to complete upon signing into our office. It is important that you bring a list of all medications that you are presently taking. You might be asked to arrive a little early for your scheduled appointment in order for us to update your patient file. Please have authorization approvals before appointment time. New patients must bring x-rays and MRI's ( if applicable ) to your first office visit.
