Dr. Anaisys M Ballesteros, DO

Patrick Azcarate, M.D., is a cardiologist at Miami Cardiac & Vascular Institute, part of Baptist Health. Dr. Azcarate specializes in general cardiology (coronary artery disease, prevention cardiology, heart failure), echocardiography and nuclear imaging, and cardio-oncology. He is fluent in English and Spanish.Prior to joining Baptist Health in 2022, Dr. Azcarate completed his internal medicine residency at the University of California San Francisco and his cardiovascular disease fellowship at the University of California San Diego. Here he grew his passion as a clinician educator for which he earned the Fellow of the Year award in 2020.He focuses on medical education to help solve complex cases through advanced diagnostic techniques and innovative treatment plans.Dr. Azcarate’s approach to patient care is to listen to his patients first and foremost, which helps forms a successful healthcare partnership. When he is not treating patients, Dr. Azcarate enjoys hiking, playing piano, traveling and pursuing photography.

Sergio Tierrablanca, D.O., is a family medicine physician at Baptist Health and is fluent in English and Spanish. Before joining Baptist Health in 2023, Dr. Tierrablanca completed his family medicine residency at Broward Health Medical Center. He earned his medical degree from the Nova Southeastern University Dr. Kiran C. Patel College of Osteopathic Medicine in 2020.Dr. Tierrablanca seeks to help future physicians as he teaches physical exam skills to Nova Southeastern University Medical students. He is also a member of the American College of Osteopathic Family Physicians, American Academy of Family Physicians and the American Osteopathic Association. As a family medicine physician, he seeks to establish long-term partnerships with families in the Miami area, helping them achieve their health and well-being goals. In his free time, Dr. Tierrablanca likes spending time with loved ones, staying up to date and attending medical conferences/lectures, playing and watching sports, watching movies at the movie theater, trying new restaurants and learning new activities such as dancing.

Damian Chaupin, M.D., is a cardiologist at Miami Cardiac & Vascular Institute. He specializes in preventing, diagnosing and treating diseases of the heart and blood vessels. In addition to his three Board certifications, Dr. Chaupin main interests are in valve disease and congestive heart failure. His clinical research studies focus on discovering improved ways to predict, detect, treat and cure cardiovascular disease.
