Hallmark Nursing Center, a Life Care Center of America facility is proud to offer a continuum of care with post-acute skilled nursing, short-term physical, occupational and speech therapies as well as outpatient rehabilitation. Rehabilitation is a focus at this facility, providing comprehensive individualized programs and services for patients of all ages. The therapy team communicates closely with the physician, nurses and social services, dietary and activities personnel to provide comprehensive care. This facility uses an interdisciplinary team approach, which involves patients and families in establishing a plan of care with the goal of returning to home and community life. The team at Hallmark believes that their patients deserve dignity, respect, and the highest possible quality of life.
