The Colorado Community Health Network (CCHN) represents Colorado's 15 Community Health Centers (CHCs) that together are the backbone of the primary health care safety-net in Colorado. It also provides access to dental and mental health services. The Colorado Community Health Network provides more than 1.8 million visits, including medical, dental, and mental health, to more than 452,000 patients. It also offers direct assistance to existing Community Health Centers (CHCs) and to Colorado communities exploring the CHC model.
