Horizons Care Center, operated by Volunteers of America, offers skilled nursing and rehabilitative services. It provides various specialty programs, including diabetic management, psychiatric services and cardiac rehabilitation. The center offers various health services, such as home visits by nurse and nurse aides, physical, speech and occupational therapies, medical social worker for patients needing intermittent post-hospitalization care, wound care, injections and catheter care. The facility also provides personal care services, including assistance with toileting, grooming, dressing, meal preparation, laundry and light housekeeping. It also provides other services, including group and home delivered meals, information and referral, caregiver training and transportation, social services and educational programs. Horizons Care Center is based in Eckert, Colo.
