Transitions Mental Health Association (TMHA) is a nonprofit organization dedicated to eliminating stigma and promoting recovery and wellness for individuals with mental illness in San Luis Obispo and North Santa Barbara Counties. Through a wide array of services, including housing, family support, employment assistance, and clinical treatment, TMHA assists over 300 individuals each year on their recovery journey.

This facility along with it's case managers are quite disengaged when it comes to managing their client's cases. There seems to be little to no pro-active action taken by the case managers...
