Shoreline Care Center

Shoreline Care Center, located in Oxnard, Calif., is the skilled nursing facility of Convent Care. It is a more than 200 bed nursing center with a rehabilitation department offering therapies, including physical, occupational and speech. It offers services, such as 24-hour skilled nursing, wound care, hospice and respite care, dental and optometry services, infusion therapy, respiratory monitoring and, pain and case management. The center provides treatment for stroke, brain injury, neuromuscular dysfunction, arthritis and Parkinson's disease. It also treats orthopedic injuries, multiple sclerosis, dementia, cardiac and pulmonary conditions and swallowing disorders and provides total knee and hip replacements. It also provides programs, including wound management, neurological deficit management, orthotic and prosthetic management, and gait, balance and fall recovery. The center also offers quarterly care conference to all patients, discharge planning, cognitive assessment and home management and caregiver training.
