Jeanne Phillips, MD - Montage Medical Group-Cardiology

Richard Gray, MD, is the medical director of Community Hospital's Tyler Heart Institute (THI). He graduated from the University of Texas Medical School at San Antonio and completed his residency at Sepulveda VA Hospital in Los Angeles and cardiology fellowship training at Cedars-Sinai Hospital in Los Angeles. He practiced at Cedars-Sinai for 18 years and was a professor at University of California, Los Angeles School of Medicine. In 1992, he was appointed professor and chairman of medicine at the University of North Dakota School of Medicine. He later became director of cardiovascular services at Health Partners and chief of cardiology at Regions Hospital in St. Paul, Minnesota and a professor at the University of Minnesota School of Medicine. In 2002 he was named director of Sutter Pacific Heart Centers in the Bay Area. He joined THI in 2009 and practices cardiology with Montage Medical Group.

Diana Monsalve, FNP, MSN, RN, is a board-certified family nurse practitioner specializing in cardiology. She received her nursing degree at University of Phoenix, Reno, Nevada, and her master of science in nursing from the University of Nevada, Las Vegas. She has more than five years experience in the cardiology field and is fluent in Spanish.
