We are now bringing our services to selected Los Angeles areasSpecialtiesIt would be helpful to have a nurse or caregiver you know you can count on to help with services to include companion care, post-operative monitoring, appointments, provide education to complicated diagnoses, addiction relapse, recovery and support, monitor for safety, provide support, and much more. We provide a service that is unique and individualized. Our nurses and caregivers can come to the home, hospital or physician's office to assure health care and wellness is a priority. The initial start-up service includes: Initial assessment Fall risk assessment Skin assessment Nursing assessment Post-Op Cosmetic Procedures Safety checklist Medication review Drug Testing Health Portfolio binder includes Emergency Contact, health alerts, blood pressure, blood sugar logs, drug testing, etc. *Education relating to nutrition, disease process, and health maintenance. *Our nurses also provide addiction, recovery, relapse support and coaching* *24 hour nurse hotline for clients with comprehension services A registered nurse will come out weekly or as needed to monitor medication adherence, diet, blood sugar levels, skin assessment, safety, make appointments, attend doctor visits, and much more. Continued Care Nursing Services partners with E-privacy to ensure all medical and health informations (HIPPA) All nurses are screened have professional liability insurance.
