HistoryCarePatrol was founded in 1993 by Chuck Bongiovanni, a medical social worker. While visiting several of his senior aged patients, Chuck realized they were living in inappropriate communities. Their individual needs had not been taken into consideration and where they were living those needs were not being met. He did some research, and Chuck realized that being in the wrong communities was detrimental to his patient's well-being. He knew there had to be a better way. He realized that families lacked the proper information to make an informed choice both by level of care and by quality of care.
