Public Health Service

Arizona Department of Health Services Division of Licensing Services, located in Phoenix, licenses and monitors health and child care facilities and providers throughout Arizona. It also conducts licensing inspections, on-site surveys and complaint investigations, and focuses on promoting safety and performance standards in facility operations and maintenance. The Arizona Department of Health Services offers a wide range of health care programs and services, such as controlling epidemics, and testing newborns for various metabolic disorder ensuring safe food and water. Arizona Department of Health Services Division of Licensing Services also monitors hospitals, nursing homes, assisted living and child care centers.

The Office of Assisted Living Licensing is operated by the ADHS Division of Licensing Services, which is a part of the Arizona Department of Health Services. The office licenses and inspects assisted living and adult foster care homes statewide. It also investigates complaints and maintains a public file for licensed health and child care institutions. The ADHS Division of Licensing Services conducts on-site surveys. The division also maintains the Office of Behavioral Health Licensing and the Office of Medical Facilities Licensing. It inspects group homes for developmentally disabled people. The ADHS Division of Licensing Services additionally performs Medicaid certification inspections.
